National Child Death Review Reporting System

Georgia will be moving to the national web-based reporting form for ALL 2009 child deaths. This online reporting system is very easy to use and user-friendly. There are fantastic features that will help committees enter data faster and easier. Training on this form will occur in Spring 2009. Information on training will be available shortly.

Please note: this form is not yet activated. Please do not submit any 2008 deaths using this national reporting form system. If you have questions or concerns, you may contact any of the CFR staff at 404-656-4200.
